I've got a victorian style 12v wired door bell in my house but looking to modify it so it could notify the various Echo Dots I have around the house.

A routine would be straightforward enough, but need a relay or similar that would get triggered when the door bell is pressed.

Doing a search, there are plenty of Wifi smart relays that switch based on a routine or via a mobile etc. I need it to work the other way round (i.e a press on the door push will trigger a routine and still sound the internal bell).

I don't want a Ring or Nest doorbell which would do this, it would look wrong on an old property!

I run a slightly different system using home assistant etc, but this may work on yours if xiaomi zigbee door sensors can integrate directly with alexa?
I have installed a relay inside my door bell that connects across the bell, so it activates when the bell operates. I have then removed the reed switch from the xiaomi door sensor and wired it across a NO contact in the relay.
Now every time the doorbell rings I get an alert via telegram with a photo from my CCTV camera!

Do you have a permanent 12v DC supply at the bell? Shelly 1 Maybe?

Yes 12v at the bell point along with the door push. Shelly 1 sounds like a good option, the blurb says it can be used to detect a door bell push. I just need to find a wiring diagram to find out how to do it :)

It has an SW (Switch) input. Wire that up to the return from your doorbell button so it goes high when the button is pressed? Try and keep the existing bell wiring so everything still works if the "smart" device dies.

Another option is to use an esp8266 like a wemos D1 mini. Put tasmota firmware on it and wire to the bell using a relay. Power it via a 12v to 5v regulator. Tasmota can be picked up directly by alexa so it could all stay locally controlled.
