What is presently regarded as the most cost effective brick/cavity/block wall in terms of which insulation and what type of block to use?

It depends what deals are on at the time. Last three or four jobs I've worked on full fill Superwall slabs were very good value. With lightweight inner blocks this gives you a U-value of about 0.30 or with medium density blocks a U-value of about 0.32. If you want to crank the U-value up to 0.28 go for an aerated inner block.
