You would typically set the waterproof layer (ie the balcony roof) at least say 75mm below the cill level, the waterproofing layer can then lap up the wall and underneath the door cill, then have your paving sat on pedestals or similar to be flush with the internal floor level, a bit like the image below. It would be unusual to have an ACO set up on a balcony floor.
