Failed Floating Floor - Tips & Advice?

Hello! Just looking for a bit of advice on where to start as we're new to all this! The vinyl kitchen floor in our new house was sagging so today we decided to investigate. We have uncovered several layers of board resting on uneven, randomly spaced planks of various types of wood. Some of the layers of wood have suffered from woodworm damage which has spread to the adjacent skirting boards. Beneath the wood is blue insulating foam which is sitting on either uneven rough concrete or stone. Obviously, it all needs to be removed before we can properly decide how best to approach replacement and repair - but any tips or advice in the meantime would be welcomed!

Woodworm presence may indicate dampness. Is that underfloor area ventilted at all?
 
State of DPM? Have you done a moisture test on any of the timbers?
 
No moisture readings taken but it felt very dry down there and certainly did not smell damp. There's a small area of ventilation above the insulation (see photo) and it looks as though there is some type of plastic sheet membrane beneath the skirting board against the wall though I can't make out where it goes without lifting up more boards.
