Floor Insulation over concrete

Hi, need some advice. so I'm insulating a stable to be made into a games room. Floor wise I'm finding different advice online about the layers needed, the concrete floor is already laid.
My thoughts are:

Concrete
DPM
100mm solid insulation
Loft chipboard TandG flooring
Finish - laminate wood etc


Couple of questions - do I need a breather membrane too anywhere ?
Can the loft chipboard layer be plywood?
Do I secure the payers together with adhesive. If so what type?

Do you also need an underlay under the laminate on top of the t+g?
 
The manufacturer of the laminate will specify. The underlay recommended for the laminate I used was slightly tacky which perhaps stops it sliding around.
 
I always put a slip layer between chipboard and insulation just thin polythene like vcl it stops the creaking between the two layers
 
If laying laminate, we have over the years tended to use a bit of PVA in the header joints to stop them from pulling open. Not necersarily essential, but does just negate issues in the future if the floor does move.
