Flooring installation (laminate on rough concrete)

I’m building a new home. I went in to check on the progress and I noticed they started to put in my laminate plank flooring. I asked them if they leveled the concrete and they said no. They said they were not informed it should have been leveled and for me to call the builder and affirm they would get paid. I did so and pointed out some (but not all areas) that need leveled. The builder told them to patch and level the floor. I can back and found that they used mortar that was in the garage (for the brick work to be done outside) to level. They only attempted to level the areas pointed out and not the entire floor. I know mortar is not appropriate material to level the concrete. They also failed to put down a vapor barrier. They only used a 2 mm foam under layer that did not cover all the concrete floor: it was 6 inches from the perimeter.

The floor isn’t level. Areas squeak and bow when walking on it. I’m afraid of integrity issues. Here are two examples of what I mean (I linked two videos above). Update: moderator will not allow me to link videos.

Should I be concerned? How should I address these issues?

They shouldn’t of laid it. They should check its flat or level. Then Discuss with you about using a smoothing compound on the screed to get the levels right
