That is such a bodge, what on earth is the point of having a level threshold when there's a step down anyway.

Anyway what should have been done was the external skin carried on up to the same level as the underside of the door frame, then a proper/longer door cill extended over it, some insultaion stuffed in the cavity, keeping then ends of the floor joists away from the wet. A healthy step down from the door cill to the patio. Get rid of the step.

I was going to board the gaps and pour concrete in to create a lintel then expanding foam between lintel and bottom of pvc frame

Is there any brickwork underneath these wood chunks? or have they just been banged into the earth? and hoping for the best.

Is the kitchen floor solid or suspended?

It may be awkward to lay bricks in there without removing the door or removing the step.

Wouldn't it be best to remove the step completely. This would give you easier access to the void making it easier to repair. The step serves no purpose anyway. It's literally the same height as the door ledge.
