A very common sight in India where electricians work practically without basic safety gear while trying to rectify a problem.
Notice the bare feet and rope tied around his waist!!!!

are you sure it's HV? If indian practice is anything like british practice four wires would generally mean it's LV stuff. Those insulators don't look anywhere near big enough for HV either.
 
I've seen worse than that in India and Pakistan.

Notice how a SWA cable is tied to the pole & how the cores are knotted onto the phase conductors.

In the markets you can buy a pole hook that's made to hook onto a line for free electricity.

They have a lot of 100 and 200kVA transformers mounted on poles along the street. A very simple method is used to indicate an overload - they let the transformer blow up.
