Look, the thread is going round in circles, we get what you're saying but if you really want to check then you'll need to open things up and see what's behind the finishes, so that will be an expense and lots of work and you might find things are afoot which need further destructive works and expense to put right so it begs the question do you want open that Pandora's box or just get it signed off and deal with any issues that may arise in the future. It seems a no brainer to me which route to take.

This.

Or it it some kind of grant work that you're not paying for and you feel pressured into accepting a shoddy job by someone who wants it wrapped up and off their desk?

It was done with a grant from the local authority accessible homes team by a builder they chose. We definitely feel pressured as the project is 6 months past the finish date and it really does feel like they are just trying to get it off the books.
Determined by who, and assessed against what standards?

Was it made safe and rectified to the required standard?

The local authority head surveyor who has been in charge of this project so I guess assessed by their standards. They gave the builder many chances to fix the problems but he never did. They fired him as they said the work was so poor, unsafe and well over schedule, the project was supposed to be finished in January but there are still major problems with it. The wet room tiles started falling off the wall when the grab rails were used in the shower for example.

Yes, the question was, if this work had been recorded as "unsafe, poor and not up to standard" what has been done to make it safe, good and up to standard?

The local authority in house building services has been to fix some of the lesser issues such as broken sockets, loose fixtures including taps and doors and fixing the radiators to the walls properly, but they have not given us a start date for the tiles and other larger problems.
